Kaoru Ishikawa was a quality improvement demigod. He trained to be a chemical engineer but somehow got diverted into industrial engineering and was on the ground floor for the so-called Japanese Economic Miracle.

Ishikawa coined the term, “friendly tools” and the fishbone diagram is arguably the easiest among the seven. The tools are “friendly” because they don’t require anything beyond the simple of statistics, and they are powerful for revealing the sources of quality issues.

The fishbone diagram, also known as the Ishikawa diagram, also known as a cause and effect diagram, is most often used as part of a root cause analysis. It's a brainstorming technique that categorizes causes of defects. The categories are often the 6Ms, which is a handy taxonomy of the sources of special cause variation:

  1. Method
  2. Machine
  3. Material
  4. Measurement
  5. Environment (Mother Nature)
  6. People (Man)

The hardest part of fishbone diagramming is keeping the brainstorming group on track. If a fishbone diagram leads a team to broad “root causes” such as…

  • management challenges
  • poor communication
  • discipline issues
  • inadequate staffing (always a favorite)

…it probably means a novice facilitator let go of the wheel and allowed the team to steer themselves into the comfortable hopelessness of, “ain’t-my-problem.”

Fishbones should produce specific, tangible causes, within the team’s span of control, which can be addressed with equally specific solutions.
